Get Started With Family Home Service Company

Thank you for your interest in working with Family Home Service Company. This application is required for all new commercial customers prior to scheduling service. Completing this form allows us to set up your account, confirm billing and vendor requirements, and ensure a smooth onboarding process. 

What This Application Is For

The New Commercial Account Application is used to:


  • Establish a commercial customer profile
  • Collect billing and accounts payable contact details
  • Identify vendor onboarding requirements (W-9s, certificates of insurance, portals, etc.)
  • Confirm payment terms and invoicing preferences


This application applies to all commercial customers, regardless of payment method or job size.

Information You Will Be Asked to Provide

Please be prepared to provide the following information:


Company & Contact Information

  • Legal business name
  • Service location(s)
  • Main business phone number
  • Primary contact name and email


Billing & Accounts Payable Details

  • Billing address
  • Accounts payable contact name, email, and phone number
  • Preferred invoice delivery method (email, portal, etc.)


Vendor / Compliance Requirements

  • Vendor onboarding or registration instructions
  • Certificate of Insurance (COI) requirements
  • W-9 or additional documentation requests
  • Purchase order (PO) requirements, if applicable


Providing complete and accurate information helps prevent delays in service and invoicing.

Payment Terms & Billing Policy

Standard Commercial Terms

Unless otherwise approved in writing:

  • Payment terms are Due Upon Receipt
  • Payment is expected at the time the invoice is received
  • Past-due balances may result in delayed or suspended service


These terms apply to all new commercial accounts by default.

If your company requires invoicing terms (Net 30), credit approval is required.
